The Culminating Survey identifies your graduates' present status and obtains the concluding feedback regarding issues of importance to your school.

The Culminating Survey is mailed with your final public relations letter 5 years after graduation (if you enroll in our 3 survey program). Graduates have the option to respond to the paper survey mailed to them (return postage paid) or follow the enclosed instructions to complete the survey online.

Submit your questions to LifeTrack (5 Yes/No and 9 Multiple Choice questions). LifeTrack will build your customized survey to fit the form as shown and mail to graduates with your public relations letter.

LifeTrack will telephone (3 attempts minimum) each graduate who did not respond by mail.

LifeTrack compiles data and sends you a personalized compilation report along with your original surveys.
