The Senior Exit Survey allows you to obtain immediate feedback from your seniors prior to graduation.

Utilize this survey to document your seniors' plans for the future, your school climate, and other critical issues related to the education your students receive.

Submit your questions to LifeTrack (6 yes/No and 9 Multiple Choice questions). The back of the survey can be utilized for open-ended questions (not tabulated in the compilation report) should you desire. LifeTrack will build your customized survey to fit the format as shown.

Surveys are sent to you to be administered prior to graduation, typically by April 30th. Completed surveys are returned to LifeTrack. LifeTrack compiles data and sends you a personalized compilation report along with your original surveys.
